Prior to your appointment with our Rehabilitation and Integrative Medicine Department, please complete and submit the following form. Thank you and we look forward to seeing you at your upcoming appointment.

  • Examples: cooperative, avoidant, indifferent, defensive, offensive, etc.
  • Examples: toy, praise, food, etc.

  • Medical History

  • If yes, please list type and general date.
  • Examples: jumps, climbing, diving, etc.

Starting Monday, April 8th through the end of September, the Illinois Department of Transportation (IDOT) will begin their road resurfacing project along Waukegan Road from Lake Cook Road to Half Day Road (IL 22).
