This form is required when a person is bitten or scratched while on VSC property. Upon submission and when applicable, a detailed Workers' Compensation form will be automatically emailed to the VSC employee for their use should they choose to seek medical attention.

Rabies Confinement Agreement Required

Due to this animal's classification as a biting animal, a rabies confinement agreement is required. This agreement must be signed electronically by the client and subsequently sent to Lake County.

June 10, 2025 - Revisions to the Illinois Animal Control Act

As of June 10, 2025, the Illinois Animal Control Act (510 ILCS 5/13 (a-10)), Ch 172.05(C) requires owners of biting dogs and cats to have their animal microchipped, at the owner's expense, if not already implanted:

Microchipping after release from confinement. At the expense of the owner, if not already implanted, a biting dog or cat shall be microchipped within 30 days after being released from confinement, and the owner shall provide written verification of the procedure to the Health Officer. The Rabies Confinement Agreement has been updated to allow for the reporting of the biting animal’s microchip number if they already have one.
Lake County Animal Care and Control will be following the State’s rabies observation and confinement timelines, where biting animals are to be confined and observed for a period of not less than 10 (ten) days from the date of the bite. For example, if a dog/cat bites on June 1^st, and the first rabies observation examination is conducted on June 3^rd, the date for the rabies observation release will be June 10^th. Historically, LCACC has imposed a confinement period for the full ten days, regardless of the date of the bite. This will no longer be the case.

Types of confinements:

Hospital Confinement: requires a biting dog/cat to be confined and observed by a licensed veterinarian within 24 hours of the bite and remain quarantined for a period of not less than 10 days from the date of bite.
Home Confinement: requires a biting dog/cat to be observed by a licensed veterinarian within 24 hours of the bite and at the end of the quarantine for a period of not less than 10 days from the date of bite.
One Time Final: requires a biting animal to be confined at home for the 10-day rabies observation and observed by a licensed veterinarian at the end of the 10-day confinement.

When the victim is a person: (when the victim is both an animal and a person,these requirements apply)

If the biting animal has a current rabies vaccination (given at least 30 days prior), a Home Confinement is required.

If the biting animal does not have a current rabies vaccination, or evidence of a current rabies vaccination cannot be provided, a Hospital Confinement is required.

When the victim is an animal:

If the biting animal has a current rabies vaccination (given at least 30 days prior), a One Time Final is required.

If the biting animal does not have a current rabies vaccination, or evidence of a current rabies vaccination cannot be provided, a Home Confinement is required.

Rabies Vaccination/Registration and Microchip are required to be completed by the end of the rabies observation, if not previously completed.

If previous rabies vaccination is set to expire within the next 30 days, a rabies certificate is required.

The Rabies Confinement Agreement has been updated to allow for the reporting of the biting animal’s microchip number if they already have one.

For dog/cat vs wildlife not captured/positive rabies results:

If the dog/cat has a current rabies vaccination (given at least 30 days prior), revaccinate and home confinement for 45 days (rabies tag/registration is NOT required for booster rabies vaccine/no tag is issued).

If the dog/cat does not have a current rabies vaccination, or evidence of a current rabies vaccination cannot be provided, revaccinate (rabies tag/registration IS required) and place under strict home confinement for 6 months.
- Then, if the dog/cat is apparently healthy after 5 months, revaccinate (rabies tag/registration is NOT required for the booster rabies vaccine/no tag is issued).

Rabies Vaccination/Registration and Microchip are required to be completed by the end of the rabies observation, if not previously completed.

If previous rabies vaccination is set to expire within the next 30 days, a rabies certificate is required.

Rabies Specimen Submission Required

RABIES SPECIMEN SUBMISSION

When there is a known bite of a person by a deceased patient within 10 days of the date of death, we MUST submit a rabies specimen (head).
A staff member needs to explain to the client that we need to submit the sample and that the client is responsible for the fee.
If we need to obtain a rabies specimen from the patient, the body must be refrigerated ONLY until the specimen is collected. The specimen MUST be collected within 48 hours of the patients’ passing.
ALL rabies specimens are to be submitted to VDL (Diagnostic Supplier: Trace First, Inc.”. Order the “Rabies FA test” (test code V-rabfa). You do not need to add a separate shipping charge.
Surgical interns and surgical assistants can remove the heads. Notify them that there is a rabies specimen that needs to be processed. The specimen must be double bagged, put in a box and placed in the ward hold REFRIGERATOR. Both the bag and box should be labeled. DO NOT FREEZE BODY PRIOR TO SPECIMEN COLLECTION.
Give the requisition form to the lab. Do NOT leave the form with the specimen inthe fridge. If the lab is not directly given the order form they will not know it is ready to send out. If the lab is closed leave the requisition form in one of the VDL bins in the ICU.
*** Submit the cremation tag with just the body and indicate that a rabies sample has been sent out on the Trusted Journey submission form so they are aware of the missing head AND so they can take steps to handle the remains SAFELY. The body can go in the freezer at this point.
Since a brain tissue sample is needed (the entire head must be sent into the state of Illinois to have brain tissue extracted and checked) the client CANNOT visit the body after collection.
They must choose for cremation through Trusted Journey or another company of their choosing.
These samples will be medically disposed of and due to many complications: U of I does not ship these part of the remains back.
In rare cases the owner may be able to pick up the rabies specimen remains at U of I (as long as the rabies results come back negative) and transport them to Trusted Journey themselves. We must call U of I and Trusted Journey to make these arrangements before the sample is shipped. This is strongly discouraged.
